Download, complete, and mail the following:

Once you obtain the City staff signature on the form, mail or deliver the completed application, a check payable to Bassett Creek WMC for the required fee (or submit payment electronically), and project attachments (project plans: one full size (paper), one 11x17 (paper), and one electronic (pdf) (jherbert@barr.com) to:
